1. 1)  to receive any and all individually identifiable health information about the past, present and future medical condition of Participant, including, but not limited to, information necessary to the care and treatment of Participant and any illness or injury Participant may have sustained;
2. 2) to authorize medical care for Participant, including, but not limited to, any and all treatment, examination, diagnosis or outpatient medical care rendered under the general or special supervision of and on the advice of any physician or surgeon licensed to practice medicine by the applicable licensing body in the state in which physician or surgeon practices.

In connection with my child or ward’s voluntary participation in activities coordinated by St. Andrew the Apostle Parish, , sponsored by the Diocese of Kansas City – St. Joseph (“Diocese”), I hereby agree, for myself, my child/ward, heirs and assigns, release and discharge the Released Parties, their officers, directors, employees, clergy members, agents, contractors and volunteers from all claims, demands and actions for injuries sustained by my child/ward as a result of his/her participation in activities coordinated by St Andrew the Apostle.  I further release and discharge the Released Parties from any COVID-19 related claim through the possible infection of my child/ward resulting from their participation in St Andrew the Apostle coordinated activities, and any subsequent infection of myself and other family members through our attendance at St Andrew the Apostle coordinated events.  I further attest and affirm to withdraw my child from St Andrew the Apostle activities under circumstances where my child may be exhibiting symptoms of COVID-19 or was considered a “close contact” as defined by the CDC, with another person who is confirmed or exhibiting symptoms of COVID-19.
